Surgeonfish and Ich
They are very beautiful and hardy fish, however they are proned to get ich very easy. A couple of things I could think of might contribute to the outbeaks. One is your retailer, if they sell you a sick fish one time then they will most likely do it again. Change your fish dealer and fully eradicate the ich in your existing tank. Second, the ick could be in a stage where you don't see it on the fish, but is still in the tank! There are three different stages as the first is one where the parasite is in the substrate and is not visible. Second, is the free swimming stage where they are floating around looking for a host. This stage is also almost undetectable as well. And the last stage is where they attach to the host and start the process all over. The last stage is where most people catch the ich and treat it. The trick is to stop it as soon as you notice it. Some triks to battle ick is to crank up your heat, turn off your lights, and do daily water changes.

Heres a vote. I had piranhas a couple of years ago mine just ate each other had 5 then in a year one. you must better at them then me.
